Cosmetic Surgery: Promises and Pitfalls™
Browlift Surgery in Dallas
PROMISES
The brow lift is a method to surgically elevate the eyebrows to an attractive and youthful
position. This reduces forehead wrinkles, eliminates excessive upper
eyelid skin, and brightens the appearance of the eyes!
PITFALLS
A good operation when indicated and properly done, however :
- Over-correction of the browlift can result in a “deer
in the headlights“ appearance.
- Most frequent complication is recurrence of the brow sag which
the reason for so many devises to hold the brows in position,
including skeletal fixation (screws in the skull), suture support, and other devices.
- Many techniques are used to correct brow sag, however there
is no single outstanding technique. In fact, “A respected
author concluded in a recent scientific article that management
of eyebrow ptosis (sag) remains a most challenging problem and
that there is no single superior solution for brow ptosis currently
available”
- Most browlifts are done at the level of the bony skull, which
places the surgeon at mechanical disadvantage (lifting skin from
the deeper layers)
- Scalp scars may be visible and often the hairline is elevated
by surgery.

OUR APPROACH
The browlift is a frequent facial procedure that we perform to
improve the appearance of the area around the eyes and the eyes themselves.
We have not been satisfied with results from procedures considered “standard” and
have developed a new approach that permits us to precisely elevate and
maintain brow position. This technique has provided a more consistent,
natural and long-lasting result.
In our experience, a more superficial approach
is ideal for brow lifts, since the brows are an appendage of the skin. Therefore
our dissection is just deep to the skin and fat of the forehead, so that
movement of the brows is most direct. In addition, we have combined
our progressive tension sutures (PTS) to the browlift, so that the brows
are lifted and anchored directly and precisely with sutures. The
forehead is advanced with PTS. This spreads the tension of the lift
over the entire forehead. The PTS firmly anchor the skin flap to the deeper tissues, eliminating the need for drains and pressure dressings. Resulting scars in most cases are barely visible.

We have found this to be a very effective approach to sagging
brows -- with a high rate of satisfaction and minimal complications.
